NEW HIRES and NEWLY BENEFIT ELIGIBLE EMPLOYEES and SPOUSES

Employees and their spouses will each create or login to their own separate accounts to maintain individual privacy, and must schedule their own individual appointments. Just follow the instructions below:

NEW HIRES and NEWLY BENEFIT ELIGIBLE EMPLOYEES and SPOUSES IN 2018, PLEASE NOTE:

  • If you participated in a screening between 08/01/18 or after, your screening points will be applied to both the 2018 and 2019 incentive plan years, and you do not need to screen again.

  • DEADLINE TO SCREEN- New Hires and newly benefits-eligible employees must screen within 60 days of your new hire date or status change date to earn an incentive.

THREE SCREENING OPTIONS:

  1. Onsite New Hire Screening Event - Schedule in Step 2 of Online Registration Process.

  2. Physician Form to screen with a Healthcare Provider - Download from your Dashboard in Step 2. NOTE: Physician Forms should only be used after medical benefits are effective. Participants who choose to screen prior to their medical effective date may be responsible for the cost of their screening if they don’t have other coverage. Previous Labs can be submitted from prior screenings back to August 1, 2018.

  3. LabCorp Voucher at a LabCorp Patient Service Center - Download voucher and instructions from your Dashboard in Step 2. NOTE: Participants must complete the online registration to provide a tobacco/nicotine use statement, when using a LabCorp screening option.

The LABCORP OPTION IS NOT AVAILABLE at the following locations whose closest biometric-certified LabCorp location is more than 25 miles from the location – PLEASE CHOOSE ANOTHER SCREENING OPTION: All St. Francis locations; Hillcrest Hospital Claremore; Cushing, OK; OK Heart Institute Grove; Hillcrest Hospital - Pryor; Hillcrest Hospital Henryetta; Seton Med Center Harker Hts., and Roswell Regional Hospital.

Texas:

  • BSA New Hire Employees and their Spouses will screen at the New Hire Walk-in Clinic in the Employee Health Department on Friday mornings, or they can choose a Provider Form in the Scheduling Step #2.

ADDRESS CHANGES - ALL EMPLOYEES: All result letters will be available on the website, and one letter will be sent by mail to the address provided by your employer. If you have had an address change and the address on your registration is incorrect, you must notify your employer to have the address changed in order for the change to appear in Bravo's system. You will be able to view your results and/or results letter online with a validated account, approximately 10 business days after your screening event, at www.bravowell.com/ardent.
