Welcome Ardent Health Services Participants!

Registration for the fall Annual Group Screening will be open July 23, 2018 - October 17, 2018. You must complete your screening by October 17, 2018.

Please be advised that screenings held onsite are full or have very limited availability. The remaining screening options are to see your personal healthcare provider or go to a LabCorp facility.

  • If you choose to use results from your personal healthcare provider, you can submit results from August 1, 2018 thru October 17, 2018. Please download the provider form found on Step 2 of your portal and have your provider complete the form. You will need to upload the completed form on your portal dashboard, or fax your form to us, by October 17, 2018.

  • If you choose to use LabCorp, please download the LabCorp voucher found on Step 2 of your dashboard. To assure availability at the LabCorp facility, follow the directions on the LabCorp Instructions to schedule a screening appointment to occur on or before the October 17, 2018 deadline. After attending your LabCorp screening, your results will be sent to Bravo electronically and there is nothing you need to submit.

Select Login and enter your Username and Password. Helpful links are available to retrieve your information, if needed.

Select Create an Account if this is your first time using this portal. This will allow you to create your account and begin the registration process.

PLEASE NOTE: Employees and Spouses must each create their own individual account and complete all steps.


  • If you participated in a screening between 01/01/18 - 7/31/18 YOU MUST SCREEN AGAIN between 8/1/18 – 10/17/18 to receive discounts for 2019.

  • If your 2018 screening took place on 8/1/18 or after, your screening points will be applied to both the 2018 and 2019 incentive plan years, and you do not need to screen again.

ADDRESS CHANGES - ALL EMPLOYEES: All result letters will be available on the website, and one letter will be sent by mail to the address provided by your employer. If you have had an address change and the address on your registration is incorrect, you must notify your employer to have the address changed in order for the change to appear in Bravo's system. You will be able to view your results and/or results letter online with a validated account, approximately 10 business days after your screening event, at www.bravowell.com/ardent.
